Radiofrequency Ablation for Varicose Veins

Varicose veins may start as a purely cosmetic concern, but when left untreated, they can cause swelling and aching in the legs, among other progressively worsening symptoms. Identifying and treating the underlying condition, chronic venous insufficiency, that causes these symptoms requires expert care and specialized knowledge. Some patients have symptomatic chronic venous insufficiency without obvious varicose veins, which is why performing a detailed venous duplex ultrasound is an important part of the evaluation.
At Prevosti Vein Center, Dr. Louis G. Prevosti brings over 30 years of cardiovascular expertise and 15 years of focusing exclusively on vein treatment. His preferred approach for treating varicose veins and venous insufficiency is radiofrequency ablation (RFA), a minimally invasive technique that eliminates problematic veins with minimal downtime and exceptional results.

What Is Radiofrequency Ablation for Varicose Veins?
Radiofrequency ablation is an advanced treatment that uses precisely controlled thermal energy to seal varicose veins from within. When the backflow prevention valves in a vein are damaged or weakened, blood pools in the extremities, causing visible veins and uncomfortable symptoms. By closing the affected veins, RFA allows blood to reroute through healthy nearby blood vessels while the treated veins are naturally eliminated by the body.
Unlike vein treatments that address only surface appearance, radiofrequency ablation targets the root cause of venous insufficiency. This comprehensive approach provides relief from symptoms such as leg heaviness, aching, cramping and restless legs. It is also the foundation for eliminating or improving the appearance of varicose veins and other visible surface veins.
How Does Radiofrequency Ablation Work?
Radiofrequency ablation uses thermal energy delivered through a thin catheter to create a controlled heating effect within the vein wall. The treatment uses radiofrequency waves to generate heat at approximately 120°C (248°F). This precise temperature causes the collagen in the vein walls to contract and the vein to collapse. A specialized type of local anesthesia called tumescent anesthesia is used to numb the area so patients do not feel any pain from the heat.
The ClosureFast™ system Dr. Prevosti uses features advanced FDA-approved technology that monitors and controls energy delivery throughout the treatment. The system's feedback mechanism achieves uniform vein closure by automatically adjusting power levels based on tissue response. This results in consistent, predictable outcomes while minimizing the risk of overheating the surrounding tissues.
Once the vein is sealed, your body's natural processes take over. Blood immediately reroutes through nearby healthy veins, improving overall circulation. Over the following months, the treated vein is gradually absorbed by your body.

Who Is a Good Candidate for Radiofrequency Ablation?
You may be an ideal candidate if you experience signs of superficial venous insufficiency, including the following:
- Leg pain, aching or throbbing
- Leg cramps, especially at night
- Leg heaviness or tiredness
- Burning or tingling sensations in the legs
- Itching, skin irritation and inflammation
- Skin darkening, thickening, or ulceration
- Restless leg syndrome
- Visible varicose veins
- Leg and ankle swelling
This treatment is particularly effective for patients with great saphenous vein and small saphenous vein insufficiency, which commonly contribute to visible varicose veins and other symptoms. Dr. Prevosti will use his extensive experience to assess whether RFA or another minimally invasive approach would best serve your needs.
Patients seeking an alternative to traditional vein stripping surgery often find RFA ideal. The procedure offers superior results compared to older surgical methods, with the added benefits of office-based treatment and rapid recovery.
What to Expect From the RFA Treatment Process
Comprehensive Evaluation
Your consultation begins with detailed ultrasound imaging to map your vein anatomy and identify the location and extent of the problematic veins. With a comprehensive and compassionate approach, Dr. Prevosti will take time to evaluate your medical history and symptoms, explain your condition, answer questions and discuss how treatment can improve your quality of life. All evaluations, treatments and follow-ups occur at our single Canton location, with no need to visit separate imaging or surgery centers.
Treatment
On treatment day, wear comfortable, loose-fitting clothing, and bring some shorts to change into before your procedure. The actual procedure takes roughly 30 minutes, but budget 90 minutes for total time in the office. Dr. Prevosti’s gentle technique, refined through decades as a heart surgeon working with delicate vessels, promotes your comfort throughout the process.
You can drive yourself to the treatments and drive immediately afterward. You do not need to bring anyone with you, but of course, you are welcome to bring someone if you wish.
After preparing the treatment area and administering tumescent anesthesia (a specialized local anesthetic that also protects the surrounding tissues from heat), Dr. Prevosti makes a tiny incision to insert the catheter. Using the ClosureFast catheter, he guides the catheter into the targeted vein and begins the ablation process. You will remain awake throughout the procedure. Once the local anesthesia is administered, most patients feel little to no discomfort. For patients that have high anxiety or a low pain threshold, pre-procedure medication can be ordered. Nitrous oxide (laughing gas) is also available. The vast majority of patients need neither for their treatment.
After Treatment
Immediately after RFA, a compression wrap will be applied to the leg. You are encouraged to walk right away, as this promotes blood flow while reducing the risk of complications. We will provide you with post-treatment instruction both verbally and written. You can drive immediately after the procedure, and continue normal light daily activities. Most patients do not need any post-op pain medication, but when needed, Advil (ibuprofen), Tylenol (acetaminophen) or Aleve (naproxen) can be taken.
Recovery and Post-Treatment Care
Most patients are cleared to return to their normal light activities immediately following radiofrequency ablation treatment. Work at a desk or computer, especially if it can be done at home, can be resumed the same day. It is recommended to take it easy the day of the procedure. Jobs that are more physical may require a day or two of rest before you can resume work. Post-op instructions and guidelines will be given to you verbally and in writing. Any questions you have will be answered.
After your treatment, you may be advised to adhere to the following:
- Compression garments: Wearing compression stockings after treatment supports healthy circulation. We usually recommend wearing compression stockings for three days. This may be modified on an individual basis.
- Activity: Short 5-10 minute walks on the day of the procedure are recommended to improve circulation. Unless otherwise directed, you can resume light exercise such as yoga and long walks two days after treatment, with strenuous exercise starting after one week.
- Follow-up appointments: Attending follow-up appointments is a vital step of the post-treatment monitoring process, as it allows you opportunities to ask any questions or address potential concerns while having the treatment area assessed for proper healing.
